If you are injured on the job, notify your supervisor immediately. Your supervisor will report all work-related illnesses or injuries requiring treatment by a physician or which result in lost time from work, or medical restrictions by providing the employee with an Employee’s claim for Worker’s Compensation Benefits and completing the Employer’s report of Occupational Injury or Illness within one working day. A flowchart outlining the procedure to seek care and to report the injury is available.

If you wish to have your personal physician treat a work injury, fill out a Physician Designation Form and have it on file at both the LAWR personnel office and the Workers’ Compensation Benefits office.
